New York State is experiencing record numbers of COVID-19 cases and extremely high hospitalizations that are stressing the healthcare system and public health response capacity.

Accordingly, New York State issued Interim Updated Isolation and Quarantine Guidance on January 4, 2022.

AFFIRMATION OF ISOLATION – This form may be used for Release from Isolation and for NY Paid Family Leave COVID-19 claims as if it was an individual Order for Isolation issued by the Schoharie County Director of Health.

AFFIRMATION OF QUARANTINE – This form may be used for Release from Quarantine and for NY Paid Family Leave COVID-19 claims as if it was an individual Order for Quarantine issued by the Schoharie County Director of Health.
